Christine,

Nolo Press of San Francisco is a highly respected source
for legal self-help information.  Their website is nolo.com .
Their book 101 Legal Forms for Personal Use has
a form for a limited power of attorney for finance.
This book costs $18 for downloadable softcopy.
I couldn't find a general purpose durable power of
attorney form on their website.

I've used Nolo Press books on Special Needs trusts
and social security disability.  They were of high
quality and quite usable.
